Services | BRIDGES - Application

We are glad that you are interested in the Wichita Children's Home BRIDGES Phase I Program (YRC II). There are several steps for you to complete in order to be considered for admission to BRIDGES. The following is a list of items necessary for you to be considered as a candidate for the program. Please review all information and begin the application process below.

Application Packet

Download and complete as soon as possible.

Goals

On the application form, write out 3 short term goals for the next six months and 3 long term goals for the next six months to two years.

Autobiography

Write a short history of your life so far. It should be 250-750 words (1-3 pages) and should include how you got to where you are now.

Once we receive these items, the Bridges program manager will contact you to set up an initial interview. Either prior to or shortly following admission, you will need to do the following:

Psychological Assessment

You will need to contact Tracy Stuever at 684-6581 extension 255 or Rob Ross at 684-6581, at extension 287.

Substance Abuse Assessment

You will need to contact Pam McLucas at 684-6581 extension 213 to schedule a short substance abuse assessment to determine if you have any alcohol or drug use issues of which we should be aware.

Health Checkup

You will need to contact you own doctor. If you have no medical doctor, you will need to contact the WCH nurse to schedule this checkup. Contact Us!

Documents

You will need to provide a copy of the following documents for our records: Original Birth Certificate, a copy of your Social Security Card, a copy of your Drivers License or Kansas Identification Card, a copy of your High School Diploma or GED, and copies of your Medical Card or Insurance Card.

Based on availability, bed space will be offered to youth after going through the application (and in some cases, browsing the assessment results listed above. If you are offered a spot in BRIDGES, you will do an "Independent Living Assessment", and be granted admission to the Phase I portion of the program.
